How to Request a New IP Address

Who should use this form?

Faculty and staff who need to connect a device (computer, printer, server, or other networked equipment) to the University’s wired network (PittNet) for the first time.

  • Students are not authorized to request static IP addresses — contact your department’s IT support.
  • IP addresses are University property and are assigned by Pitt Digital only.

Before You Start — Gather This Information

✓ Checklist — Have these ready before opening the form

  • Your Pitt username, phone number, email, department, building, and room number
  • The MAC address (IEEE address) of the device — see “How to Find Your MAC Address” below
  • The port address label from the wall network jack where the device will plug in — see “How to Find Your Port Address” below
  • The hostname you want to give the device (short, descriptive name — e.g., mylab-pc01)
  • The type of device (PC, Mac, printer, server, virtual machine, etc.) and its operating system

How to Find Your MAC Address (IEEE Address)

Device Steps
Windows PC or Laptop
  1. Press Windows key + R, type cmd, and press Enter.
  2. In the Command Prompt window, type ipconfig /all and press Enter.
  3. Find your active Ethernet adapter (e.g., “Ethernet adapter Ethernet”).
  4. Look for the line that reads “Physical Address” — that is your MAC address.
    Example: Physical Address. . . . . . . . : 3C-A9-F4-01-2B-7E
Mac (macOS)
  1. Click the Apple menu  in the top-left corner and select System Settings (macOS Ventura and later) or System Preferences (older versions).
  2. Click Network, then select your Ethernet connection from the list.
  3. Click Details… (or Advanced… on older macOS), then select the Hardware tab.
  4. Your MAC address is shown next to “MAC Address” or “Ethernet ID.”
Printer
  1. On the printer’s control panel, look for a Reports, Information, or Print Info menu.
  2. Print a Configuration Page or Network Summary page.
  3. The MAC address appears under “Network Configuration,” “Hardware Address,” or “MAC Address.”
Server or Virtual Machine Windows Server: Use the same ipconfig /all steps as Windows above.
Linux / Unix: Open a terminal and type ip link or ifconfig -a and look for “ether” or “HWaddr.”
VMware / Hyper-V VM: In the hypervisor management console, open the VM settings and look under Network Adapter for the virtual MAC address.

How to Find Your Wall Port Address

Where to look:

The Port Address is printed on the label attached to the wall network jack (the rectangular port where your Ethernet cable plugs into the wall). It looks like this:

The format is: Building Code – Switch/Panel ID – Port Number

  • Ocl = Building abbreviation (3 letters)
  • cl728 = Network closet / panel identifier (up to 8 characters)
  • 0012 = Port number (4 digits)

Completing the Form — Step by Step

Step Form Section What to Do
1 Open the Form Open a web browser and go to:
techforms.pitt.edu/TechRequest/IPRequestNew.aspx
You do not need to log in before opening the form.
2 Your Contact Information Enter your Pitt username. The form will attempt to fill in your name. Then complete: phone number, email address, department, room number, building, affiliation (select Faculty or Staff), and campus. All starred fields () are required.
3 Request Service For If this device is yours: Check the “Same as Requestor” box. This section will collapse and no additional information is needed.
If requesting for someone else: Leave the box unchecked and enter the device owner’s name, contact info, department, room, and building.
4 Connection Use Choose the option that best describes your device:
Dedicated IP — for a single desktop, laptop, printer, or server (most common)
Multiple IPs — if one device needs more than one IP address
Laptop — if your laptop moves between different campus buildings
Shared — for conference rooms or addresses reserved for future use
Virtual Machine — for VMs running in VMware, Hyper-V, or similar
5 Host Name Enter a short, descriptive name for your device. Examples:
mylab-pc01   nurs-print02   engr-server03
Rules: No spaces. No dots. No personal names. Pitt Digital will automatically add the domain (e.g., .pitt.edu) — you do not type it here.
Tip: Use your department abbreviation + device type + a number.
6 Third Level Domain
(optional)
Most users: leave this blank. Pitt Digital will assign a domain.
If your department requires a specific subdomain, enter only the label — for example, enter hs for hs.pitt.edu, not the full address.
⚠ Do not type “univ.pitt.edu” — the form will show a validation error if you do.
7 Port Address Copy the label exactly as shown on your wall network jack (e.g., Ocl-cl728-0012). Be sure to use the port that has your Ethernet cable plugged into it. See “How to Find Your Port Address” above if you need help locating this.
8 IEEE Address
(MAC Address)
Enter your device’s MAC address in the format xx-xx-xx-xx-xx-xx (using dashes, not colons). See “How to Find Your MAC Address” above for step-by-step instructions by device type.
9 Device Type & OS Select your device type from the dropdown: PC, Macintosh, Printer, Server, or Other.
Select the operating system: Windows, Macintosh, Unix, Other, or Not Applicable (use “Not Applicable” for printers and devices without a traditional OS).
10 Installation Assistance Select Yes if you would like a Pitt Digital network technician to assist with connecting the device. Select No if you can connect it yourself once the IP address is assigned.
11 Additional Information Use this field for anything else Pitt Digital should know: special requirements, urgency, multiple MAC addresses, VLAN needs, or context about how the device will be used. More detail helps Pitt Digital process your request correctly and quickly.
Submit Review all entries, then click Submit. You will receive a confirmation email at the address you provided. Pitt Digital will contact you when the IP address has been assigned — typically within 1–3 business days during normal business hours.
