Forms are a custom entry type designed to standardize data collection across your notebooks and team. Form data can be aggregated from multiple entries into a single CSV export, allowing for streamlined data analysis and reporting processes.

Who Can Use Forms?
- Forms that are published from your Form Center can only be used within notebooks that you own.
- All users who have editing rights to your notebooks will be able to add your published form(s) as notebook entries and contribute to data collection.
- Form Center can only collect data from forms that are used within notebooks that you own. If a form entry is copied to another notebook that you do not own, the form’s data will not be included in the CSV export.
- Note: You must own at least one notebook, or be assigned as an Account Administrator, in order to access Form Center.
Who Can Manage Forms?
All Notebook Owners can access Form Center to create forms for their notebooks, publish forms for data collection, and export form data as a CSV file.
Account Administrators can create and manage forms on behalf of notebook owners.
